OK. I think I understand now. You can do this, but it will require the use of the MenuPage module.
Create a new wiki page (that will become your base menu).
- For the "plain" menu items, just use regular Tiki wiki links.
- For the "fly-out" menu items, use the regular menu plugin. Each region will need its own menu.
Something like this:

*((HOME)) *((ABOUT)) **Another Link **Another Link *Regions **{menu .....} This is the menu for USA **{menu .....} This is the menu for Canada
Then, use the MenuPage module to assign the wiki page menu to your site. See the docs for details.
